It's been a while since I've felt like posting here, but your question and situation is all so familiar. The advice given is about the best you can get, you want to make sure the pain doctor is seeing is about pain management, understands you have chronic intractable pain. and will work with you to keep you comfortable as much as he is humanly possible to do.

On the one hand, you have these lawmakers who consider chronic pain to be like suffering with hemorrhoids, they don't understand the pain is there 24/7/364.25, even if it's not always at a +14 pain level. Although that's usually what you have to list your pain level on your first visit, just to get their attention. Be sure the PM is paying attention to what you're saying, and the staff knows your situation as well as he does.

Unfortunately there are not only drug addicts out there trying to get meds for recretional use, but the DEA uses underhanded tricks like planting agents with falsified records to try to bust the pain doctors. It's a lot easier to put a case together when the client keeps records of his transactions, even if he has no way of knowing he's being led into a sting. So when you go for your first visit, don't expect much in the way of chemical help, a reputable PM will talk to you about your condition, you may get a refill if you were referred from a general practitioner, but don't count on it. In fact if the PM's willing to load you up with a lot of narcs, that should sound some alarms.

Usually the first visit will focus upon a definite diagnosis, if you don't already have one, and then some talk about what most people would consider alternate medicine - needles in the spine, electric stimulators implanted to 'dull' the nerves, psychotherapy, etc. But after a few visits, and it shouldn't be more than a couple months, if nothing works the PM should be willing to start you on opiod medications. If you've already tried things like cervical injections and the like, be sure to tell him about it and the effectiveness of the treatments. Offer all the info you can, be up front with the guy, and be patient. You may not feel relief for a while, but after a few months you should be able to tell if this is the right doc for you or not. If you don't feel like you'll get anywhere with him, tell him so and see if he'll let you out of the pain contract if there is one. You can check out other pain management centers while you're under contract, just make sure both parties know who you're seeing and don't accept pain meds from both doctors unless you are positive they know what you're taking. Not only is mixing different pain meds dangerous, it's illegal and may impede your getting treatment in the future.

I didn't see the part about the internet pill places until after I posted this. Although some are 'psyeudo-legal' most are scams, years ago you used to be able to buy some pretty potent meds from overseas pharmacies without a prescription, but since the mainstream media got ahold of the story, the game has changed. What's normally involved is paying a cyber-doctor an exorbentant amount for him to write you an Rx after you describe your symptoms, without so much as a single office visit, and that gets you a month's worth of meds at a slight to heavy markup, then the process repeats the next month. In all honesty you're bettter off finding a pill ID site and buying the pills teenagers steal from their grandparents and sell. There may be some sites out there that haven't been uncovered yet, but they will be eventually, then you'll be left high and dry with no meds. Given that, on top of the endless scams and mailing list trading, it's just not worth it. Stick with a qualified physician who sees you face to face so he can monitor your health, as some of these drugs can cause serious organ damage.
